Care By Your Side & Services is an approved Medicaid provider. We are apart of CAP (Community Alternatives Program) a medicaid funded program and we specialize in non medical home care services. This In-Home nursing support care program was designed to service eligible members based on medical necessity determination for children who are 21 years of age and under.
Our CAP services include:
CAP/C: Community Alternatives Program for Children is a program that provides home and community-based services to children through 20 years of age who are at risk of institutionalization. Our CAP/C services include but are not limited to personal care, respite care, transfers, and specialized care for children with developmental disabilities or medically fragile conditions.
CAP/DA: Community Alternatives Program for Disabled Adults (CAP/DA) is a program that provides home and community-based services to adults who are at risk of institutionalization. Our CAP/DA services include but are not limited to personal care, light housekeeping, meal preparation, and medication reminders. We also provide specialized care for clients with specific needs such as those who are developmentally disabled, medically fragile, or those who are wheelchair bound.
Our CAP services are designed to help individuals maintain their independence and dignity in the comfort of their own homes. Our caregivers and companion sitters are carefully selected and trained to provide the best possible care, and are fully vetted, insured, and bonded. We also conduct regular checks and evaluations to ensure that our caregivers and companion sitters are meeting our standards.
